$22 for initial certificate, $10 for each additional copy; fee waivers available for those experiencing homelessness from local homeless service providers.

Provides copies of birth certificates for those born in the state of Utah. Birth years 1951 to present can be requested in the office. Prior to 1951, only Utah County records are available. Birth records for other Utah counties prior to 1951 can be obtained at the office for an additional fee.
