Eligibility

All patients, insured, uninsured, medicaid. Patients with family incomes of less than 200% Federal Poverty Level can qualify for a sliding fee discount. Must meet income guidelines. Accepting new patients.

A small co-pay will be requested; in most cases will be a fraction of the total cost of the visit.

Salt Lake County, UT
Eye care services are provided by the Mobile Clinic. The mobile eye clinic collaborates with Charity Vision to provide free eye exams for prescription lenses and can also provide vouchers for free to low-cost lenses. Currently out once per week at the same sites as the mobile medical clinic. During the school year, appointments are reserved for schools and children in Title I schools.
