Application process

A newborn may be safely relinquished at any Utah hospital open 24/7. The infant should be left with hospital staff wearing proper identification or a name badge. Visit the website for a list of hospitals statewide and the optional medical questionnaire form found in the FAQ Do I have to leave my name.
