Provides The National Diabetes Prevention Program to help lower the risk of Type 2 Diabetes in higher-risk individuals. Program participants learn to make better food choices and increase physical activity. The goal is to lose 5 - 7 % of the participant's body weight and get 150 minutes of physical activity each week.

Provides a variety of health oriented services to assist the community, including birth and death certificates, international travel advisories, substance use disorder counseling and crisis intervention, immunizations, prenatal home visitation, tuberculosis screening, chronic disease management, WIC, child seat inspections, and STD screening. Operates out of 6 different locations around the Bear River area.
