Application process

This is not a licensing office but typically visited to purchase a Motor Vehicle Report (MVR), reinstate a driving privilege, or pick up a lost or surrendered license. Purchase the MVR online and/or call Customer Service at the main number to reinstate and track a lost or surrendered license.

Fees

Driver history record (MVR - Motor Vehicle Report) and reinstatement of driver license can be purchased at this office. Payment by Cash, Check, Money Order, Debit Card and Credit Cards are accepted at this location for reinstatement of license.
