Varies based on local program. Generally families must meet at least one of the following criteria: low income, pregnant and under the age of 21, history of child abuse, neglect, or substance abuse, current tobacco use in the home, low parental educational achievement, a child in the home with developmental delays or disability, or a household member who has served in US armed forces.

An evidence based prenatal/postnatal home visitation program for first time moms, beginning during their pregnancy where a nurse provides one on one support, advice, and information needed to have a healthy pregnancy, until the child turns two years old. Home visits are tailored to the needs of the families served and can take place in the family’s home, an office, park, or wherever the family is most comfortable.
