Application process
Call for an initial screening. Walk-ins are welcome but wait times may be longer without a scheduled appointment.
Fees
Fees are set on a sliding scale and funding assistance may be available. A "Self Pay" option is also available. Accepts Medicaid and other private insurance. Please call for more information.
