To get connected to a care coordinator, call or email. Families can also reach the care coordination team with questions via the "Contact" tab on the website.

Care coordinators can help families get connected to early childhood resources throughout Utah. Depending on family needs, they may provide contact information for resources or call and schedule appointments for the families. After providing referrals, care coordinators will follow up with the family 1 month and 3 months later, with possible continued follow up after that. Also provides a searchable database of early childhood resources.
