Eligibility

Must provide information demonstrating medical need and ability to care for medical alert or assistance dog. Must be over the age of 18 to sign as primary caregiver of the service dog.

Application process

Visit website for online application. A committee meets twice annually to review applications. Once an applicant is approved, they will be matched and provided with a timeline for training with their service dog.
