This program provides services to people living in nursing homes who want to move out into the community and/or help divert individuals from having to enter an nursing home. The program identifies the needs of participants, helping them make the transition and connects them to appropriate agencies and support systems in the community.

Providing organization

Utah Independent Living Center
A nonprofit, non-residential agency that assists persons with disabilities to achieve greater independence by providing services and activities that enhance independent living skills. Provides advocacy, independent living skills instruction for people of all ages, a nursing home transition financing program, specialized information and referral, assistive technology equipment loans, life skills education for youth, general recreational activities/sports, and peer counseling.
